Option #1
    Registration forms submitted online in Word document format to modelcongress@aysf.org.

• There is no fee for submitting your application online.
• Please download registration form, complete, and email to modelcongress@aysf.org.
• Add modelcongress@aysf.org to your address book and all safe sender lists immediately to ensure delivery of all important and time-sensitive documents)
• Note that your application is NOT complete until you have received confirmation that it has been received via email. Confirmations are sent on a weekly basis and if you have not received yours within a week of sending, call Aurelia Figueroa, program director at (408) 375-0894 to follow-up on your confirmation status.

    Print and complete the online registration form and send via postal mail to:

Registration forms submitted via postal mail are subject to a $10 application fee payable by check or money order to the American Youth Scholarship Foundation. This fee should be included with your registration form.
